Process Safety Performance Analysis
Having completed comprehensive process safety reviews for its operations in the North Sea, Middle East and Kazakhstan, this oil and gas operator was faced with a mountain of findings requiring analysis to identify trends, understand common causes and provide the basis for corporate and local action plans.
A small team of specialists was assembled, including expertise in process safety, auditing, data analysis, HSE systems and culture. The team moved to an undisturbed off-site location to live with the data, develop a methodology, test it with independent experts, and then crunch through the entire data set. Multiple crosschecks and verifications were performed, taking different combinations and perspectives on the data. Emerging messages and ideas for graphical presentation were sense tested with senior operations and engineering staff, this also serving to obtain their “buy-in” ahead of the final report and feedback.
Common causes and combinations of causes were identified both globally and locally, with league tables indicating priority action areas. A formal and detailed report was prepared, and a summary presentation was well received by members of the sponsor group. A sub-set of the team subsequently worked with the organization to develop and expedite action plans.
